New Approach to Enhance Near Miss Incident Reporting

Near miss incidents form the basis of Heinrich’s domino theory which is free lessons learnt incidents. Reporting of near miss incidents is an important tool in preventing incidents. These are incidents which had potential but resulted in no injury/illness, damage, production loss, or harm to company reputation. It will address the various methods being used in different industries to overcome the barriers in near miss incident reporting to enhance the safety factor of an organization.

N.Vasudevan is working in field of HS&E for past 18 years. He is qualified fire Engineer with post graduation certification in safety. He started his career as Engineer Trainee in Hindustan Petroleum Corporation Visag refinery and promoted to Deputy Manager- Fire & Safety.

He joined Kuwait national Petroleum Company MAA as Safety Engineer in the year 2001 and was promoted to Sr. Safety Engineer in February 2006. He has wide range of experience in field and has conducted various specialized training program in the field of HSE. He has also presented technical papers at various forums including at ASSE Kuwait PDC 2009. He has been actively involved in development of various procedures including Incident investigation, Emergency preparedness plan, Planning and execution of HS&E budgets & work permit system.

He is a certified HAZOP Expert & Head Professional development committee ASSE Kuwait Chapter.

He is a certified safety Professional CSP USA .

Presenter – Mr. Dinesh Kumar Dutta

Dinesh Kumar Dutta is working in field of Health, Safety & Environment (HSE) for past 19 years. He is a qualified Fire Engineer with Post Graduation in Industrial Safety. He started his career as Engineer Trainee in year 1990 in Hindustan Petroleum Corporation Vizag Refinery, India and promoted to Deputy Manager- Fire & Safety before Joining Kuwait National Petroleum Corporation (KNPC), Kuwait in year 2002.

He joined KNPC in May 20002 and was promoted to Sr. Safety Engineer in February 2006. He joined Oil Sector Services Company (OSSC), a subsidiary of Kuwait Petroleum Corporation (KPC) in August 2007 and is presently working as Sr. Technical Support Engineer.

He has wide range of experience in field of HSE and has conducted various specialized training program in the field of HSE. He has also presented technical papers at various forums including ASSE Kuwait PDC conference during 2007 & 2009. He has been actively involved in development of various procedures including Incident investigation, Emergency preparedness plan, Unit Turnaround HSE Plans, Work Permit System etc. and development of HSE Management Systems.

He is an active member of ASSE, Kuwait Chapter and is a Certified Safety Professional (CSP), USA.
